those residents who require intensive
rehabilitative care, an array
of therapy programs are coordinated
and offered in and from the facility’s
on-site therapy unit and gymnasium.
When therapy goals are attained
and the resident prepares to
return home, the staff conducts
an at-home evaluation to assist
the resident in making the transition
to home as easy as possible.
